Answers
1a) (b) No. You must untap Kill Switch.
1b) (b) only killswitch untaps - During the untap step, everything untaps simultaneously. If an effect is preventing one or more permanents from untapping, they won't untap at this time, even if the very nature of the untapping ends the effect.

2c) 8 life. Use it when you put it into play- that's 2 life. Turn 2 - take off a fading counter, use it - that's 4 life. Turn 3 - take off a fading counter, use it - that's 6 life. Turn 4 - put the take off a counter on the stack, put use it on the stack, use it resolves - that's 8 life. Put it in the Graveyard. Under Fifth edition rules, you could not use the ability of a permanent until you had payed its upkeep cost. However, now, things that used to be "upkeep costs" are now triggered effects that go on the stack at the beginning of the upkeep step. You can then put "Tap: gain 2 life" on the stack. So that even when the chamber has zero counters on it, you can use it before you have to put it in the graveyard (for being unable to take a fading counter off of it).

3.Converted mana costs
3aAnswer: a) usually zero. However, some cards do have effects that yield a "copy" of a card. For example Dance of Many and more recently, Dual Nature all create tokens that are "copies" of permanents. Given that they're copies, all characteristics should be included in the xerox effect, including mana cost. So we should say "non-copy tokens have converted mana cost = 0".
3bAnswer: a) zero
3cAnswer: b) 1 generic mana. Converted mana cost does not include "extra" costs imposed by other spells or abilities (or permanents)
